Records Management file request form

This form is for the sole use of UO departments needing access to their own inactive, non-permanent records housed by Special Collections and University Archives (SCUA).  Because originating departments retain legal custody of the records, direct access to these records is limited to the originating department and to SCUA. Other UO units needing access to another department’s inactive records should contact the originating unit first. Members of the general public who wish to gain access to non-permanent records should contact the Public Records Office.  
  • As a reminder, we are unable to fulfill more than 15 file request per month for each department.
  • We attempt to hand deliver records within 5 business days.
  • If your file request does not fit into the form, please email with the file request.
