Proxy Program 

The University of Oregon Libraries proxy program allows UO faculty members to designate one person to act as representative when checking out materials from the Libraries. Faculty members should not share their online library services log-in credentials with their proxies, or anyone else. 

Faculty members who wish to designate a proxy should complete this proxy request form. Your proxy will be authorized when they present the signed form and a photo ID card at any UO Libraries checkout desk. The individual's proxy status will expire after one year, or your designated expiry date, whichever is sooner.

Please note that by designating another individual to act as your proxy, you are authorizing that person to check out materials on your library account. You are responsible for the replacement costs, service charges, and any overdue fines that accrue for materials checked out to your account, whether by you or your proxy. If your proxy leaves UO or is no longer working for you, please contact Knight Library Access Services as soon as possible to have them removed from your account (541-346-3065, ecirc@uoregon.edu).
