SUBJECT SPECIALISTS: Use this form if you want to:

• catalog a free Internet site
• add or order the backfile of a journal
• purchase an electronic journal, database, or another electronic source
• switch a title from print to electronic only
• add electronic access to an existing print resource
• request or set up a trial

When completing this form, please provide as much information as you can. Requests will go into a queue and will be processed like any other request/order on the basis of first in/first out. Please inform us about those requests that require priority or rush processing.
